You could also rather than complete the questionnaire, have buttons that are a currated shorthand, like LDS, catholic, or Yale skull and bones, AI reflecting the values of those organizations.

Some, like traditional Indian names, which I suspect might still greatly vary from region to region, since India is more like a series of kingdoms, a la Holy Roman Empire. Also, not just how the created them, but what they meant to people might also vary.
Having said that, this is only practical for a topic since it's likely well documented, being so legally important for texting the continuity of identity and inheritance, but also probably studied academically and religiously. Thus AI should have a lot to work with that is early verifiable. There's probably even books on it.
The good part through, is what implications you can draw regarding the future, including the effects on births and families. Also, might suggest effects regarding human migration and regional intermarriage and cultural mixing/collapse.
